The following categories will be awarded and the form can be downloaded below note the closing date is Sunday 7 August 2016

Senior Sports Personality (SSP)This award is made to a sportsperson 18 years of age or over as at 1 July 2015 residing in Midlothian, who has achieved outstanding success in their sport during the preceding year.

Junior Sports Personality (JSP)Nominees for this award should be under 18 years of age as at 1 July 2015 and be a resident or representative of Midlothian. Nominees should have achieved outstanding success in their sport during the preceding year. Previous winners may not be nominated for the Junior Sports Personality award again.

Senior Team Award (STA)

This award is made to a senior (over 18s) team. Nominees must be based in or represent Midlothian and should have achieved outstanding success during the preceding year.

Junior Team Award (JTA)Nominees for this award should be under 18 years of age as at 31 March 2015 and must be based in or represent Midlothian and should have achieved outstanding success during the preceding year.

Coach of the Year (COY)

This award is made to a coach who is a resident of Midlothian or who coaches a Midlothian team or individual. Nominees should have achieved outstanding success as a coach during the preceding year.

Service to Local Sport (SLS)

This award is presented to an official or volunteer who has made an outstanding contribution to local sport over a lengthy period. All nominated persons must be a resident or be involved with a club within Midlothian.

Volunteer of the Year (VOY)

This award is made to a volunteer who in the preceding year has made an outstanding contribution to the Active Schools Programme in Midlothian.
